Description

This sterile-compounded solution licensed by the Government of AJ&K provides high-concentration potassium chloride (KCl) 10% (1.34 mmol/mL) in 0.9% sodium chloride base. Reserved for severe hypokalemia (<2.5 mmol/L) under cardiac monitoring, it enables rapid electrolyte correction while maintaining osmolarity (2000 mOsm/L). Not for direct IV push – fatal if administered undiluted.

Primary Clinical Indications

  • Critical Hypokalemia: Serum K⁺ <2.5 mmol/L with ECG changes (U-waves, ST depression)

  • TPN Supplementation: As additive to parenteral nutrition

  • Diuretic-Induced Losses: Refractory to oral replacement

  • Metabolic Alkalosis: With concomitant chloride deficiency


Critical Administration Protocol

Life-Threatening Risks

☠️ FATAL IF:

  • Administered undiluted (cardiac arrest within minutes)

  • Infused >40 mEq/hour without monitoring (ventricular fibrillation)

  • Given via IV push (even 1-2mL can be lethal)

  • Used in renal impairment (CrCl <20 mL/min)


Precautions

⚠️ Absolute Contraindications:

  • Hyperkalemia (>5.0 mmol/L)

  • Acute renal failure

  • Addison’s disease

  • Concurrent potassium-sparing diuretics

⚠️ Adverse Effects:

  • Phlebitis (peripheral administration)

  • Hyperkalemia (overcorrection)

  • Tissue necrosis (extravasation)
